Lebanon VA Medical Center, Primary Health Facility

Known for its award-winning patient and employee satisfaction, the Lebanon VA Medical Center was in need of a new facility to support the growing demand for integrated primary and behavioral health services. 

This new addition is integrated seamlessly into the historic medical center campus and adds 20 exam rooms, eight consult rooms, and two group care rooms.

Building on History

This new Primary Care Facility builds on AE Works’ history of working for the Department of Veterans Affairs, on the Lebanon VA Medical Center campus, since 2007. Primary care and mental health support service teams work together in spaces that were thoughtfully designed to create a calming, comforting environment through daylighting, color palette, acoustics, and materials. Design and construction work was coordinated with other active projects at the Medical Center. The project was designed to support a 2-story vertical expansion and meet all VA Physical Security Design requirements.

Supporting Patient Aligned Care Teams (PACT)

This facility is home to the first Patient Aligned Care Team (PACT) clinic at the Lebanon VA Medical Center. It hosts six PACTs, each of which can see more than 1,000 patients. PACTs operate as a multidisciplinary group of healthcare professionals, including doctors, nurses, pharmacists, and mental health providers, to provide comprehensive care to patients. The teams emphasize patient-centered care, fostering strong relationships between healthcare providers and patients to promote better communication and health outcomes. This is an important project for the Department of Veterans Affairs as it exemplifies the future of the care we give our veterans.
